Why do I need extraction of Tooth?

“Why do I need extraction of tooth?” is the common question that we encounter  as a dentist. But,wait before you blame us.As,a dentist, I would recommend saving the tooth as much as possible,but sometimes we have to make the decision to take out some teeth to improve the health and function of oral cavity.And if Dentist wants to remove the tooth,then the predominant reason is patient’s negligience. Some of the reasons why dentist advise extractions are:

  1. Badly damaged tooth because of dental caries or trauma.
  2. Impacted or mal-positioned teeth.
  3. Crowded teeth and extraction because of Orthodontic reasons.
  4. Failed root canal treated tooth.
  5. Severe gum disease.

What to expect with tooth extractions?

General dentists are trained to do simple as well as surgical extractions. Complicated teeth because of root anatomy and proximity to nerves are some of the reasons patients will be referred to Oral surgeon. Before the extraction, dentist will take thorough medical history, examine the treatment plan, check radiographs, explain patient all the available alternatives and finally required consents for extraction. If one or two teeth are being taken out, local anesthesia is preferred. Topical anesthetic gel is applied at the injection site wait for couple of minutes and administers injection and it can be either a buccal infiltration or a nerve block. Once desired numbness is achieved tooth will be taken out with different surgical instruments. Patients will be awake during the process and feels pressure sensation during the procedure but no pain. Popping and cracking sounds can be heard sometimes as the tooth is coming out. Sometimes if the tooth breaks or badly decayed tooth, gums are reflected and surrounding bone is removed in order to facilitate extraction. Deeper sedation is recommended if more teeth are being taken out or if patients cannot tolerate extraction with local anesthetic.

What to expect after extractions?

  1. Pain and soreness after the numbness is gone. Dentist will prescribe you pain medication as needed.
  2. Antibiotics may be given sometimes if there is risk of secondary infection. Prophylactic antibiotics are also prescribed sometimes.
  3. Dentist will make sure bleeding is controlled before they release you. Dentist will also make you bite on a wet gauge and apply pressure for next 30min.
  4. Avoid sucking on anything and anything that creates negative pressure in your mouth.
  5. Do not use straw when you drink.
  6. Drink lots of fluids and go with soft food for next 24hrs.
  7. No smoking and hot food and drinks for next 24-48hrs.
  8. Sometimes dentist might recommend using ice pack for next 24-48hrs to reduce the swelling. Place cold pack in the area 20 minutes on and 20 minutes off.
  9. Heat packs are recommended sometimes after 2-3 days to reduce stiffness.
  10. Avoid strenuous activities and lifting heavy stuff for next 5-7 days.
  11. Sometimes stitches are placed during extraction and they will dissolve and fall off after a week.
